Congregation Etz Chaim (“CEC”) In-Person Shabbat Sign-Up Protocol

(1) All members must sign-up individually. You may not sign-up for family or friends. If you have attended services in-person once during a particular month, please give others a chance to sign-up before you attend a second time in the same month.
(2) Be considerate of fellow congregants. If you sign-up, please attend unless you become ill. If you cannot attend, please delete your sign-up before Shabbat, if possible, so the slot opens for someone else.
(3) By signing-up to attend an in-person Shabbat service the CEC member acknowledges and agrees that:

· you have NOT been diagnosed with or tested positive for COVID-19;

· you do NOT have a fever and do NOT exhibit symptoms of COVID-19 or other flu-like symptoms,

· you have NOT knowingly been exposed to anyone suspected or confirmed to have COVID-19 within the past 14 days;

· you will disclose to CEC if you test positive for COVID-19 and assist with congregational contact-tracing efforts;

· you will wear a mask at all times inside the building and will practice social distancing in accordance with CEC and all applicable governmental orders and CDC and Georgia Department of Public Health (“DPH”) guidelines; and you will only sit in designated seats.  Seats that are "taped-off" with blue tape CANNOT be used;


· while CEC is implementing certain additional cleaning and sanitation measures in our building, we do not represent or guarantee that our building is safe to use. By entering the building you assume the risk that you may be exposed to COVID-19 and you are responsible for ensuring your own safety and practicing appropriate health safety measures recommended by the CDC and the Georgia Department of Health.
